  12.  Veterans cried about how they were treated, worried whether they
  13. would live to see Christmas and reported identity mix-ups that had
  14. nurses mismatching drugs at the VA hospital in Nashville, which has
  15. some of the nation�s longest wait times to see doctors.

  17.  These were the stories Juan Morales, director of the Tennessee Valley
  18. Healthcare System for the US Department of Veterans Affairs, heard
  19. during a town hall meeting Monday afternoon. About 90 people filled a
  20. small room, holding up their hands and waiting for a chance to speak.
  21. Their list of complaints was long, ranging from reports of administrative
  22. staff ignoring them while talking on cellphones to grievances about
  23. doctors abruptly canceling appointments or misdiagnosing illnesses.
  24.  
  25.  The meeting was the third of four briefings Morales has scheduled with
  26. veterans in the wake of a congressional investigation about delays veterans
  27. faced nationwide trying to see doctors. While established patients in
  28. Middle Tennessee had an average wait time of three days, according to
  29. a government audit, veterans needing to see a specialist didn�t get in
  30. the door for 71 days on average. Those were veterans primarily needing
  31. to see ophthalmologists, podiatrists and pain specialists.

  33.  The wait time for a new patient to see a specialist now averages 61 days,
  34. Morales said.
  35.  
  36.  But an established patient from Goodlettsville is also concerned about
  37. wait times. Robert Morgan worries he has cancer that won�t get diagnosed
  38. and treated soon enough. After he complained of stomach pain and bathroom
  39. problems, he said, it took a month to get a CT scan when nodules were found
  40. in his kidneys and liver. He said he thinks he may have colon cancer that
  41. has metastasized.
  42.  
  43. 'Not expeditious'
  44.  
  45.  �They said I needed another CT scan, which was scheduled for two weeks later,�
  46. Morgan said. �Trust me, that�s not expeditious. I�ll die, and the reason I�ll
  47. die is because I don�t receive timely care. There are a lot of guys in this
  48. room who are sicker than me, and it will probably happen to them, too.�
  49.  
  50. Morales said he would have a staff member check on the situation.
  51.  
  52.  �I�d appreciate it if you�d do something, because I�d like to be here come
  53. Christmas,� Morgan answered. �Right now, I don�t think I�m gonna be.�
  54.  
  55.  Misty Hollars told how she could not get her father, Miles Hollars, transferred
  56. from a hospital in Franklin, Ky., to the VA hospital in Nashville. The family
  57. had to go to Vanderbilt University Medical Center, she said, asking whether the
  58. VA system would cover that hospital bill.
  59.  
  60.  Roger Morris of Clarksville brought with him the names of the nurses he said
  61. put him into a �bloody bed,� neglected to put an identifying wristband on him
  62. and then got the identities of patients mixed up and brought them the wrong
  63. medications.

  65. �Wrong medications can kill somebody,� Morris said. �Do you understand me, sir?�
  66.  
  67.  He said he asked to be discharged so he could go to another hospital, and the
  68. nurse took his bloody IV tube out and laid it on a food tray.

  70. �Can you explain that, sir?� Morris asked.
  71.  
  72.  Morales apologized and promised to investigate, saying, �First of all, I�m
  73. sorry for the experience you had. If that�s what happened, that�s not acceptable.�

  75. 'Stellar expert care'
  76.  
  77.  Jim Haggar said he had received �stellar expert care" recently at the Nashville
  78. VA hospital and had never seen �a cleaner hospital room in my life."
  79.  
  80.  �I just want to make sure the people who do a fantastic job are getting a little
  81. bit of notice as well,� he said.

  83.  But Haggar did suggest that the VA outsource hospitality training for its front
  84. -line staff, with the goal of making them as customer-centric as people who work
  85. at the Cleveland Clinic.

  87.  Morales said the Nashville hospital was adding staff after receiving federal
  88. approval to bring on an additional 323 personnel and build space for more
  89. examination rooms. He said the area�s expanding population had made it difficult
  90. to meet the growing demand for care. He admitted that it might be difficult to
  91. fill some sub-specialty positions, such as pain physicians.

  100.  The Veterans Affairs Department and other government agencies are not doing
  101. enough to help women who served in the military, even as their number is rising
  102. dramatically, according to a new report.
  103.  
  104.  The report, released Wednesday by the Disabled American Veterans, identified
  105. serious gender gaps in virtually every program serving veterans, including health
  106. care, job training, finance, housing, social issues and combatting sexual assault.
  107.  
  108.  The advocacy group�s report blamed most of the deficiencies on a disregard
  109. for the needs of female veterans, saying the VA and other agencies focus on
  110. �the 80 percent solution for men who dominate (veterans affairs) in both numbers
  111. and public consciousness.�
  112.  
  113.  A sharp increase in reporting of military sexual trauma is an illustration of
  114. problems that require �radical change� at the VA and throughout the military,
  115. the report says.
  116.  
  117.  �At a time when the number of women veterans is growing to unprecedented levels,
  118. our country is simply not doing enough to meet their health, social and economic
  119. needs,� said Joy Ilem, DAV�s deputy national legislative director. Female veterans
  120. �deserve equal respect, consideration and care as the men who served, yet the
  121. support systems are ill-equipped to meet the unique needs of the brave women who
  122. have defended our country,� she said.

  124.  The DAV report closely tracks an Associated Press review in June that found
  125. serious shortcomings in how the VA cares for female veterans returning from
  126. Iraq and Afghanistan, many of them of child-bearing age.

  128.  The AP review found that nearly one in four VA hospitals does not have a fulltime
  129. gynecologist on staff, and that 140 of the 920 community-based clinics serving
  130. veterans in rural areas do not have a designated women�s health provider, despite
  131. a goal that all clinics have one.

  133.  Female veterans of child-bearing age were far more likely to be given medications
  134. that can cause birth defects than were women being treated through a private doctor,
  135. the AP found.
  136.  
  137.  The VA cared for about 390,000 female veterans last year at its hospitals and
  138. clinics � far fewer than the 5.3 million male veterans who used the VA system in
  139. fiscal year 2013. But the number of women receiving care at VA has more than
  140. doubled since 2000. The tens of thousands of predominantly young, female veterans
  141. returning home have dramatically changed the VA�s patient load, and the system has
  142. yet to fully catch up.

  144.  While the number of male veterans is expected to decline by 2020, the number of
  145. female veterans is expected to grow dramatically, to 11 percent of the veteran
  146. population, the report said.

  148.  Dr. Carolyn Clancy, the VA�s acting undersecretary for health, said the VA will
  149. consider all of the report�s 27 recommendations on topics including health care,
  150. education, job training and sexual assault.

  153. of female veterans and their supporters at the Capitol on Wednesday. The VA is working
  154. to ensure that all clinics and hospitals have a women�s health provider onsite, she

  156. available at the VA.

  241. US military healthcare is "a system in which scrutiny is

  244. Scrutiny, "the military system has consistently had higher

  379. in Park Forest, Illinois.

  383. when Victory Center employees attempted to persuade Wrana to

  386. in an attempt to resist paramedics.

  411. a priority, but saving the planet is Job 1. Solar panels and
  412. windmills can be more important than the touch of a healing

  415.  The department early on set up an Office of Green Management
  416. Programs designed to �help VA facilities nationwide recognize
  417. opportunities to green VA, and to reward innovative �green�
  418. practices and efforts by individual facilities and staff within
  419. the VA.� This sometimes means paying more attention to greening
  420. the department and saving the polar ice caps than to health care.

  422.  In the department�s words, it adopted a far more important
  423. mission to �become more energy efficient and sustainable, focusing
  424. primarily on renewable energy, energy and water efficiency,
  425. [carbon-dioxide] emissions reduction, and sustainable buildings.�

  427.  The green office isn�t merely a desk and telephone tucked away
  428. in the dark corner of a nondescript government building. It�s a
  429. substantial undertaking, with all the luxury, bells and whistles
  430. of a bureaucracy that means business. Eric K. Shinseki, who
  431. resigned as secretary in the wake of the VA scandal of the sin
  432. of omission, traveled the country to boast of the green initiative.

  434. at a half-million-dollar windmill project at the Massachusetts
  435. National Cemetery. �Nationally,� he said, �VA continues to expand
  436. its investment in renewable sources of energy to promote our
  437. nation�s energy independence, save taxpayer dollars, and improve
  438. care for our veterans and their families.

  571. provide what is among the most comprehensive accounts yet of
  572. how high-level VA hospital employees conspired to game the
  573. system. It shows not only how they manipulated hospital wait
  574. lists but why�to cover up the weeks and months veterans spent
  575. waiting for needed medical care. If those lag times had been
  576. revealed, it would have threatened the executives� bonus pay.

  578.  What�s worse, the documents show the wrongdoing going unpunished
  579. for years, even after it was repeatedly reported to local and
  580. national VA authorities. That indicates a new troubling angle
  581. to the VA scandal: that the much touted investigations may be
  582. incapable of finding violations that are hiding in plain sight.

  584.  �For lack of a better term, you�ve got an organized crime
  585. syndicate,� a whistleblower who works in the Texas VA told
  586. The Daily Beast. �People up on top are suddenly afraid they
  587. may actually be prosecuted and they�re pressuring the little
  588. guys down below to cover it all up.�

  590.  �I see it in the executives� eyes,� the whistleblower added.
  591. �They are worried.�

  593.  The current VA scandal broke in Phoenix last month, when a
  594. former doctor at a VA hospital there became the first whistleblower
  595. to gain national attention. The doctor�s allegations of falsified
  596. appointments�and veterans dying while they waited for treatment
  597. �unleashed a wave of similar claims from VA employees nationwide.
  598. In Cheyenne, Wyoming, Chicago, and Albuquerque, more VA
  599. whistleblowers came forward claiming that the same fraudulent
  600. scheduling was being used in the hospitals where they worked.
  601. At last count, the VA inspector general�s investigation had
  602. expanded to 26 separate facilities.

  604.  The torrent of claims led to Senate hearings, calls for VA
  605. Secretary Eric Shinseki to resign, multiple investigations
  606. and President Obama�s own public statement last week. Paul
  607. Rieckhoff, founder and chief executive officer of Iraq and
  608. Afghanistan Veterans of America (IAVA), believes that even
  609. more revelations are coming.

  611.  �This newest case just further illustrates that the scandal
  612. is much more far reaching than most people realize,� Rieckhoff
  613. said, �Phoenix was just the tip of the iceberg. Scandal has
  614. become the new normal, it�s the status quo at the VA right now.�

  617. the root causes of the VA crisis have remained murky. New
